SMART Surgery, Samadi Modified Advanced Robotic Technique "The perfect union between advances in robotics and technology".

SMART (Samadi Modified Advanced Robotic Technique) surgery is an innovative technique for performing minimally invasive robot-assisted laparoscopic prostatectomy (LAP), which aims to achieve complete removal of the cancerous prostate, preserving sexual potency and eliminating post-surgery urinary incontinence problems.

The Hospital Metropolitano de Santiago (HOMS), located in Santiago, Dominican Republic, is home to the Samadi-Homs-Robotic Institute, which has a robotic surgery program with special application in prostate cancer, laparoscopic surgery assisted with the Da Vinci Xi surgical system, the most advanced surgical robot in the world.

This surgery was developed by Dr. David Samadi, a urological oncologist and expert in robotic surgery for the treatment of prostate cancer, who is based at the Samadi-Homs-Robotic Institute as director of the institute’s surgical program.

Homs has been using robotic surgery for a decade, making this state-of-the-art technology available to all patients, positioning itself at the forefront of surgical treatment in the Dominican Republic and the world.

“The goal of SMART surgery is to reduce surgical time to less than two hours and hospital stay recovery to 24 hours,” says Samadi, who has performed more than 9,000 robotic laparoscopic surgeries on patients with this carcinoma in the prostate, bladder and kidney.

The specialist emphasizes that SMART surgery is a procedure that uses the latest advances in automation and computerized technology to remove the prostate affected by the tumor.

Procedure

Samadi explains that during the procedure it is possible to have a 3D view of the surgical field through the Insite Vision system. This is a great advantage over traditional surgery and the 2D view of laparoscopic surgery. High-definition video cameras provide 10X to 15X magnification for greater depth perception and a clearer field of view.

Advantages of SMART surgery

The robotic surgical capability provided by the Da Vinci Xi system offers several advantages over the traditional laparoscopic surgical approach to treating prostate cancer, compared objectively in the following areas:

  • Clinically superior results

The Da Vinci electronic and sensitive system and one-centimeter diameter surgical instruments allow the surgeon to perform high-precision movements within the pelvic cavity. This means that malignant tissue can be removed with unprecedented efficiency and ease. This increased accuracy equates to a reduced likelihood of relapse due to loss of cancerous tissue.

  • Risk reduction at complications

Surgery is always a medical procedure with inherent risk. However, Samadi emphasizes that, for cancer treatment, surgical options may be the most effective option in eradicating malignant tumors without the painful side effects of chemotherapy and radiation therapy.

The robotic surgeon, who is included in Castle Connolly Medical America’s Top Doctors list, states that among the most serious risks associated with surgical treatment is the possibility of infection at the incision sites.

These infections are often the main causes of serious complications that can hinder the ability to recover quickly and adequately from a procedure. The da Vinci system greatly mitigates the risk of infection in several ways. First, the incision size is actually significantly smaller with robotic surgery than with the traditional procedure.

  • Minimizes scarring

Da Vinci robotic surgical technology optimizes the chances of a faster recovery time compared to traditional surgical prostatectomy. The smaller incisions required to manipulate the operating arms heal faster, require fewer stitches and are less vulnerable to rupture or infection.

The high-precision movements of the robotic arms allow for the cleanest removal of malignant tissue. Such precision also results in a more targeted approach that reduces damage to healthy tissues during the operation. The design of the operating robotic arms allows the entire procedure to be performed without the need for operating tools against the incision walls.

  • Faster recovery period

The size of the incision needed to perform a traditional prostatectomy produces significant scarring in the patient’s lower abdomen. The robotic prostatectomy procedure allows incisions that can heal with little or no visible scarring. By minimizing the overall size of the incision needed to remove the cancerous tissue, this risk of scar tissue related complications is greatly mitigated.

Samadi-Homs-Robotic Institute

Dr. Samadi, who is recognized as one of America’s foremost experts in robotic oncology, has dedicated much of his life to early prevention and helping men of all ages overcome prostate cancer in his role as director of the Samadi-Homs-Robotic Institute.

The urologist stresses the importance of early detection. “There is sufficient evidence that early detection of prostate cancer decreases mortality, as effective treatments are available for the disease diagnosed at earlier stages.”

He emphasizes that by finding any abnormality at its earliest stage, the chance of the cancer spreading can be reduced, increasing the chances of treating and even curing it. He urges men to have regular PSA blood tests and prostate screening.

Dr. Samadi recognizes that thanks to the vision of Homs and the leadership of Dr. Rafael Sánchez Español, Dominicans have at their disposal the best surgical technology in the world.
